Labraddodle Puppy Application Form

Congratulations! If you are filling out this application then you have thought through everything pertaining to owning a puppy. This puppy will become a cherished member of your family for many many years. The purpose of this application is to help us select the right puppy that compliments your lifestyle. Please put as much information as you want and if you want to phone Gail and ask any question please do so at 403-684-3419.

Applications are registered on a first come first serve basis secured by a non-refundable $350. deposit/booking fee. Once we have received your application form via email we will contact you. If you have not heard from us within 24 hours please email prairiedoodles@gmail.com or phone Gail at 403-684-3419. You can pay for this deposit either by paypal (you do not need an account to do this as I will send you a bill via paypal and you can then pay by credit card) or you can send me a cheque payable to Gail Groeneveld, Box 231, Blackie, AB. T0L0J0, Canada.

As soon as we receive your deposit and application you will be placed on the list of the desired litter. We will send information, and send photos weekly until you receive your puppy. We will be available anytime for any question about anything pertaining to dogs and puppy's for the lifetime of your puppy/dog.

When the puppy is 6 weeks old we require the balance of $2150. to be paid either by certified cheque, money order or paypal. Shipping is extra and varies depending on destination. Approximately $250. – $350. throughout North America.
